PART TWO - File Official Form 121 - Statement of Social Security Number

1.    Complete Official Form 121 - Statement of Social Security Number. You must provide the entire SSN.

2.    File electronically using Bankruptcy -  Miscellaneous - Statement of Social Security Number (Form 121)
